Reorganizes the README “Usage” section to describe cross-provider capabilities (guided generation, image inputs, tool calling) before diving into provider-specific setup details.
Changes:
- Replaces the early Apple Foundation Models usage example with a new “Guided Generation” section applicable across providers.
- Adds new “Image Inputs” and “Tool Calling” sections, including an image-support matrix and tool execution delegate example.
- Moves the Apple Foundation Models section later and removes now-redundant provider-specific notes.

Recent PRs have steadily filled out our feature matrix across providers (as well as adding a new Open Responses adapter).
This PR makes some holistic changes to the README so that everything reads more legibly and coherently.
