General Changes
Updates to support v3.6 of the protocol. The 'legacy' version of the ui pool data provider was removed. We no longer support v2 in the UI, and the latest ui pool data provider is backwards compatible, so we just need one version.
Some misc. cleanup items to remove hardcoded addresses and chain ids
