Overview
Documents TASK-258127: outbound_throttle_messages and outbound_throttle_connections can be set in a host stanza for per-MX-host rate limits (connections at connect time; messages once bound to an MX).

Updates list host as a valid scope in the options summary, adds both options to ref-host, and expands the two throttle reference pages with additive enforcement alongside binding/domain/global throttles, a separate host fallback chain (binding_group::binding::host → … → host), examples, shared token-bucket semantics, and a note that cluster-wide limits still use the cluster_outbound_* options. The 5.3.0 changelog records the feature.
