Changes
The form has a clear top section containing only: Name, Status, and Material Type,
Only the top three fields have asterisks,
Reimbursement Request # field is moved to the bottom of the form in an "Additional Information" section,
Ensured that the form works the same, so validation was changed (doesn't error with the new optional fields).
