Pick/ban map selction for BO3 during warmup #533

During comp games, teams pick/ban maps to play.
Issue
Currently, pick/bans are done off server and nominate/rtv is used to swap maps. This lets teams know ahead of time which map the opposing team intends to play and is not well integrated into the game.
Solution
Implement a map picker during warmup that lets teams pick/ban maps in order, then save the map selection and play them in the order selected.
