This follows on from #67, and adds a separate notebook that describes how to convert to DICOM with Bio-Formats, while simultaneously adding custom DICOM metadata.
This was discussed a few weeks ago with @fedorov, @dclunie, and @erindiel.
The JSON examples illustrate what can be done, but are not necessarily representative of what should be added. I can certainly update/modify the JSON presented here if there are more realistic scenarios that should be addressed.
