Updated index.html and added the HTML form based on the instructions.

Form Details:

Customer Name: Required (min 2 characters).

Email: Required (valid email format).

Color: Required (must pick 1 of 3).

T-Shirt Size: Required (XS, S, M, L, XL, XXL).

Technical: Built with HTML only (no CSS or JS) and no form action included.

Testing & Quality: I tested the inputs and achieved a 100 Lighthouse accessibility score. I hit this result by doing some final refactoring directly in DevTools to make sure everything was structured correctly.
