[Fusion] Improve OperationPlanner performance

• We had a bug in the determination of the best direct lookup, since the KeyTransitionVisitor was not being reset, so if the first one required a transition it and all the following ones wouldn't be chosen as the best direct lookup, which exploded search space.
• Another issue is that the behavior of the PriorityQueue is undefined if two items have the same priority. To make the behavior deterministic and prevent loops, I've introduced a PlanNodeQueue that adds an incrementing tie-breaker to each entry that ensures that items with the same priority are dequeued FIFO.
