Allow multiple inputs and allow page specs to refer to multiple documents

Add --output flag to standard flags. This allows psjoin to output to a file, and other programs to accept multiple inputs.

Then, add syntax to page specs for referring to an input document by number, e.g. N/ before the page number.

This allows us to implement pdfinterleave.
