error entering a user manually

• choose classlistManager (after logging in as lisa)
  http://michaelegage.duckdns.org:8080/webwork3/courses/4/instructor/classlist

• click "add users" button

• choose add users manually from the pop down.

• enter username: gage -- tabbing moves you to the last blank (role) since all of the intermediate ones are closed

• choose role:instructor

• press "add this user and close"
  result: DB exception parameters needed ?

I suspect some of the parameters of this widget weren't set correctly, if that's soI'd appreciate an explanation of how (and where) to modify them so that this widget works
