bug: Audio ducking triggered by notification doesn't bring volume back up

Bug description

I dunno if there's a setting that I missed, or maybe it's app related, but when a media sound is playing in any app (YouTube, music apps, video players, etc.) if a notification is received during the time, the volume decreases for the notification sound (Audio Ducking) but it doesn't come back up to the original volume after notification is gone. Even the volume control buttons don't help with that. Doesn't matter if audio is played on phone speaker or headsets.

Steps to reproduce

Step 1. Make sure phone is not on vibrate only or mute
Step 2. Play something with media sound (music, video, etc.)
Step 3. Receive a notification
Step 4. Audio is decreased and it wouldn't go back on until some unspecified circumstances happen (I think receiving another notification)

Relevant log (logcat/build log)

Logs sent as file. I played a song in my music app (PowerAMP), while listening via Bluetooth headphones, my friend messaged me from Telegram, the sound stayed decreased.

By disableling the Audio Focus completely via external means, the issue goes away since there's no audio ducking anymore

Mine is off should it be turned on

I turned it on Still didn't help
I went back to Adaptive playback and it was turned off
I tried again Adaptive playback won't stay on

OK I've found more
The sound only drops on what you are listening to
Like if you are using YouTube and the Volume drops
You can start up the like of VLC and sound is normal
Another thing when on Speaker phone you can't adjust the Volume at all

How do I do this turning off Audio Focus please?

Pausing the player and playing again seems to fix volume.
