Roadmap doesn't show progress for last 8 months

Issue

The development roadmap is an important document to demonstrate to the community and prospective community members the journey Mauro is on and how it is making progress.

Suggested remedy

1. Annotate the items from the roadmap with the release they were delivered in or target release where this is confirmed
2. Where a partner is contributing to delivery (e.g. NHS Digital funding DOI) annotate incomplete items as active and the partner leading
3. Retrofit previous achievements into the roadmap, so the road shows where we have come from and where we are going
