Extend package manager to use github repositories.

Phase One:
As a user, I should be able to publish a package specifying only the name of the package, the version number, and a github repository. The package manager client should take care of cloning the repository, and building a package from its contents.

Phase Two:
Move all Dynamo nodes to package repositories.

Phase Three:
Setup for #12
